    Basic studies in microwave remote sensing

    Scattering models were developed in support of microwave remote sensing of earth terrains with particular emphasis on model applications to airborne Synthetic Aperture Radar measurements of forest. Practically useful surface scattering models based on a solution of a pair of integral equations including multiple scattering effects were developed. Comparisons of these models with controlled scattering measurements from statistically known random surfaces indicate that they are valid over a wide range of frequencies. Scattering models treating a forest environment as a two and three layered media were also developed. Extensive testing and comparisons were carried out with the two layered model. Further studies with the three layered model are being carried out. A volume scattering model valid for dense media such as a snow layer was also developed that shows the appropriate trend dependence with the volume fraction of scatterers

    Presumed choroidal metastasis from soft tissue myoepithelial carcinoma

    Purpose: To report a case of presumed choroidal metastasis from soft tissue myoepithelial carcinoma and highlight challenges in its diagnosis. Observations: A 52-year-old man was referred with a two-week history of photopsia in his left eye. His background medical history included known soft tissue myoepithelial carcinoma metastatic to his bone, lung, liver and chest wall. A large, raised, yellow choroidal lesion was identified nasal to and abutting the optic disc. This lesion demonstrated growth 1 month after presentation. The patient died with widespread metastatic disease 5 months after initial presentation. Conclusion and importance: Soft tissue myoepithelial carcinoma can rarely metastasise to the choroid and present as a rapidly-growing, yellow, echodense tumour with serous retinal detachment. MRI brain can assist in tumour evaluation and monitoring progression, while immunoperoxidase stains and molecular testing can assist with diagnosis.     The Cognitive, Affective, and Somatic Empathy Scales (CASES): Cross-Cultural Replication and Specificity to Different Forms of Aggression and Victimization

    A psychometrically sound measure of empathy that captures its multifaceted nature is critical in furthering research on empathy. The only instrument that assesses three domains of empathy together with positive and negative valence empathy is the newly developed 30-item cognitive, affective, and somatic empathy scales (CASES). The current study examines the cross-culture generalizability of CASES in Hong Kong and explores links between empathy and different forms of aggression and peer victimization. A sample of 4,676 Hong Kong youth (62% male) completed CASES, alongside measures of reactive/proactive aggression and multidimensional peer victimization. A subsample of youth (n = 2,321–2,464) and their parents completed additional instruments for testing the validity of CASES. We replicated most of the concurrent, convergent, and discriminant validity findings in the original development of CASES. Proactive aggression was most strongly linked to affective empathy, whereas reactive aggression was most strongly linked to somatic empathy. Differential associations were revealed between subscales of CASES and forms of peer victimization. Findings provide cross-cultural generalizability for a brief self-report instrument that captures the multifaceted nature of empathy.     Disease gravity and urgency of need as guidelines for liver allocation

    One thousand one hundred and twenty-eight candidates for liver transplantation were stratified into five urgency-of-need categories by the United Network for Organ Sharing (UNOS) criteria. Most patients of low-risk UNOS 1 status remained alive after 1 yr without transplantation; the mortality while waiting was 3% after a median of 229.5 days. In contrast, only 3% of those entered at the highest risk UNOS 5 category survived without transplantation; 28% died while waiting, the deaths occurring at a median of 5.5 days. The UNOS categories in between showed the expected gradations, in which at each higher level fewer patients remained as candidates throughout the 1-yr duration of study while progressively more died at earlier and earlier times while waiting for an organ. In a separate study of posttransplantation survival during the same time period, the best postoperative results were in the lowest-risk UNOS 1 and 2 patients (88% combined), and the worst results were those in UNOS 5 (71%). However, a relative risk cross-analysis showed that a negative benefit of transplantation may have been the result in terms of 1-yr survival for the low-risk elective patients, but that a gain in life extension was achieved in the potentially lethal UNOS categories 3, 4 and 5 (greatest for UNOS 3). These findings and conclusions are discussed in terms of total care of patients with liver disease, and in the context of organ allocation policies of the United States and Europe

    Primary Choroidal Lymphoma Diagnosed with 27-Gauge Pars Plana Vitrectomy Choroidal Biopsy

    Currently, transvitreal fine-needle aspiration biopsy is the most widely used tissue biopsy technique in cases of suspected intraocular lymphoma due to its relative simplicity and low trauma. The small sample produced, however, may be inadequate for diagnostic and prognostic analyses due to mechanical artefacts, insufficient material, or sampling errors. Small case series have demonstrated choroidal biopsy via vitrectomy to be safe and effective. With smaller-gauge vitrectomy instruments, visual recovery is rapid, and post-operative inflammation and conjunctival scarring is minimised. Furthermore, smaller-gauge instrumentation does not appear to affect the diagnostic yield of biopsies for intraocular lymphoma in vitro. We report a case of primary choroidal lymphoma successfully diagnosed with 27-gauge pars plana vitrectomy choroidal biopsy. Case Presentation: A 72-year-old female presented with a 6-month history of painless blurred vision in her right eye. Fundus examination revealed a large pale choroidal mass centred on the posterior pole with overlying exudative retinal detachment. Enhanced depth imaging optical coherence tomography revealed a markedly thickened choroid with an undulating appearance. B-scan ultrasonography demonstrated diffuse, smooth thickening of the choroid, and retrobulbar extrascleral hypoechoic nodules. A 27-gauge pars plana vitrectomy was performed and choroidal biopsy taken. Histopathologic, immunohistochemical, and flow cytometry studies confirmed a diagnosis of extranodal marginal zone B-cell lymphoma. Systemic workup found no evidence of systemic lymphoma. As such, the patient was diagnosed with primary choroidal lymphoma. She underwent intensity-modulated external beam radiotherapy with subsequent resolution of disease. Conclusions: Primary choroidal lymphoma can be safely and effectively diagnosed via 27-gauge vitrectomy choroidal biopsy

    Towards GPU Utilization Prediction for Cloud Deep Learning

    Understanding the GPU utilization of Deep Learning (DL) workloads is important for enhancing resource-efficiency and cost-benefit decision making for DL frameworks in the cloud. Current approaches to determine DL workload GPU utilization rely on online profiling within isolated GPU devices, and must be performed for every unique DL workload submission resulting in resource under-utilization and reduced service availability. In this paper, we propose a prediction engine to proactively determine the GPU utilization of heterogeneous DL workloads without the need for in-depth or isolated online profiling. We demonstrate that it is possible to predict DL workload GPU utilization via extracting information from its model computation graph. Our experiments show that the prediction engine achieves an RMSLE of 0.154, and can be exploited by DL schedulers to achieve up to 61.5% improvement to GPU cluster utilization

    Trimmer: Cost-Efficient Deep Learning Auto-tuning for Cloud Datacenters

    Cloud datacenters capable of provisioning high performance Machine Learning-as-a-Service (MLaaS) at reduced resource cost is achieved via auto-tuning: automated tensor program optimization of Deep Learning models to minimize inference latency within a hardware device. However given the extensive heterogeneity of Deep Learning models, libraries, and hardware devices, performing auto-tuning within Cloud datacenters incurs a significant time, compute resource, and energy cost of which state-of-the-art auto-tuning is not designed to mitigate. In this paper we propose Trimmer, a high performance and cost-efficient Deep Learning auto-tuning framework for Cloud datacenters. Trimmer maximizes DL model performance and tensor program cost-efficiency by preempting tensor program implementations exhibiting poor optimization improvement; and applying an ML-based filtering method to replace expensive low performing tensor programs to provide greater likelihood of selecting low latency tensor programs. Through an empirical study exploring the cost of DL model optimization techniques, our analysis indicates that 26-43% of total energy is expended on measuring tensor program implementations that do not positively contribute towards auto-tuning. Experiment results show that Trimmer achieves high auto-tuning cost-efficiency across different DL models, and reduces auto-tuning energy use by 21.8-40.9% for Cloud clusters whilst achieving DL model latency equivalent to state-of-the-art techniques

    Client Service Receipt Inventory as a standardised tool for measurement of socio-economic costs in the rare genetic disease population (CSRI-Ra)

    The measurement of costs is fundamental in healthcare decision-making, but it is often challenging. In particular, standardised methods have not been developed in the rare genetic disease population. A reliable and valid tool is critical for research to be locally meaningful yet internationally comparable. Herein, we sought to develop, contextualise, translate, and validate the Client Service Receipt Inventory for the RAre disease population (CSRI-Ra) to be used in cost-of-illness studies and economic evaluations for healthcare planning. Through expert panel discussions and focus group meetings involving 17 rare disease patients, carers, and healthcare and social care professionals from Hong Kong, we have developed the CSRI-Ra. Rounds of forward and backward translations were performed by bilingual researchers, and face validity and semantic equivalence were achieved through interviews and telephone communications with focus group participants and an additional of 13 healthcare professional and university students. Intra-class correlation coefficient (ICC) was used to assess criterion validity between CSRI-Ra and electronic patient record in a sample of 94 rare disease patients and carers, with overall ICC being 0.69 (95% CI 0.56–0.78), indicating moderate to good agreement. Following rounds of revision in the development, contextualisation, translation, and validation stages, the CSRI-Ra is ready for use in empirical research. The CSRI-Ra provides a sufficiently standardised yet adaptable method for collecting socio-economic data related to rare genetic diseases. This is important for near-term and long-term monitoring of the resource consequences of rare diseases, and it provides a tool for use in economic evaluations in the future, thereby helping to inform planning for efficient and effective healthcare. Adaptation of the CSRI-Ra to other populations would facilitate international research

    New best1 mutations in autosomal recessive bestrophinopathy

    PURPOSE: To report the ocular phenotype in patients with autosomal recessive bestrophinopathy and carriers, and to describe novel BEST1 mutations. METHODS: Patients with clinically suspected and subsequently genetically proven autosomal recessive bestrophinopathy underwent full ophthalmic examination and investigation with fundus autofluorescence imaging, spectral domain optical coherence tomography, electroretinography, and electrooculography. Mutation analysis of the BEST1 gene was performed through direct Sanger sequencing. RESULTS: Five affected patients from four families were identified. Mean age was 16 years (range, 6-42 years). All affected patients presented with reduced visual acuity and bilateral, hyperautofluorescent subretinal yellowish deposits within the posterior pole. Spectral domain optical coherence tomography demonstrated submacular fluid and subretinal vitelliform material in all patients. A cystoid maculopathy was seen in all but one patient. In 1 patient, the location of the vitelliform material was seen to change over a follow-up period of 3 years despite relatively stable vision. Visual acuity and fundus changes were unresponsive to topical and systemic carbonic anhydrase inhibitors and systemic steroids. Carriers had normal ocular examinations including normal fundus autofluorescence. Three novel mutations were detected. CONCLUSION: Three novel BEST1 mutations are described, suggesting that many deleterious variants in BEST1 resulting in haploinsufficiency are still unknown. Mutations causing autosomal recessive bestrophinopathy are mostly located outside of the exons that usually harbor vitelliform macular dystrophy-associated dominant mutations
